Independent Verification Worksheet Introduction
Approximately 30% of all
Free Applications for Federal Student Aid (FAFSAs) filed are selected
for a review process called verification by the U.S. Department of
Education. Your
FAFSA has been selected for this process. In this process, the Office of
Financial Aid is required by law to
compare your FAFSA information with signed copies of your (and your
spouse's, if you are married) 2006 federal tax forms, W-2 forms/earning
statements, tax schedules, other financial documents, etc.
The law says the Office of Financial Aid has the
right to ask you for this information. If there are differences between
your FAFSA and the financial documents, the Office of Financial Aid may
send corrections electronically, to have your information reprocessed.
Complete this verification form and submit it to the Office of Financial
Aid immediately along with the
required documentation, so that your financial aid will not be delayed.
You will be
notified if you need to complete this form. Please be sure to print,
